Do you know that some Medicare Advantage plans are specifically designed to treat diabetes, cardiovascular and breathing conditions?
If you know someone who has Medicare and one of these diagnoses, please share this article with them. Many Medicare plan holders are eligible for a plan created to help manage their condition and don’t know it. In 2018 in Maricopa, Pima and Pinal counties, there are plans to treat diabetes, cardiovascular and breathing conditions. Please help me spread the word.
These are HMOs, Health Maintenance Organizations, most of which have $0 monthly premium, $0 copayment for primary care visits, and low specialist co-payment. It includes prescription medicine and lower maximum out-of-pocket cost than regular HMO plans. These plans have proven to be cost effective for its enrollees.
Another unique feature is that eligible persons can switch to the appropriate plan anytime, not just between October 15 and December 7, the Annual Election Period. The change can be made now.
There are three types of Medicare special needs plans (SNPs): (1) Chronic Condition SNP, (2) Dual Eligible SNP, (3) Institutional SNP. Like all HMO plans, you have to use network providers for all care, unless it’s an urgent or emergency matter.
If you have questions on these or other Medicare related coverage options, you can contact Medicare at 1-800-MEDICARE, or contact the plans directly. I will also be happy to assist you if you have questions in Maricopa.
