PGCSC introduces ‘Bridges of Care’
Pinal-Gila Council for Senior Citizens is introducing a new program entitled “Bridges of Care.” The purpose of the program is to help address the impact that COVID-19 has had on the older adult community, 60 or older. The Arizona Division of Aging and Adult Services has received federal funds for the COVID-19 pandemic response. These funds are being distributed as a one-time allocation to support PGCSC, the local Area Agency on Aging, to increase its capacity to coordinate care transitions for clients affected by COVID-19. The term “care transitions” refers to the movement patients make between health care practitioners and settings as their condition and care needs change.
Individuals enrolled in the new program will be assigned a care transition coach for a 30-day timeframe. The coach will provide outreach and education on possible in-home and community-based resources that can help support individuals as they adjust to the changes COVID-19 has imposed on their lives. A dedicated coach will take an individualized approach with each participant on their health care journey. To find out more, contact Pinal-Gila Council for Senior Citizens at 520-836-2758 or 800-293-9393.
